Attendance: Missed sessions are not refunded without 48 hours' notice. Make-up sessions are subject to availability.

Drop-off & Pickup: Children must be dropped off and picked up by an authorized adult. We are not responsible for participants before or after their scheduled session.

Behavior: Participants who repeatedly disrupt the group or endanger others may be asked not to return. No refund will be issued.

Liability Waiver & Indemnification

This is a legally binding document. Please read the entire waiver before typing your name.

⚠️ Please read this entire document carefully. By typing your name below you are giving up certain legal rights including the right to sue for injuries arising from ordinary negligence. Do not sign if you do not understand or agree to these terms.

Description of Activities

The Program includes: cultural education activities, cooking and food preparation, art and craft projects, movement and physical activities (including dance, capoeira, and traditional cultural movement), games, music, and use of kitchen equipment including knives and heat sources.

Assumption of Risk

I acknowledge that participation involves inherent risks including but not limited to: cuts or burns from cooking activities, falls or minor injuries from movement activities, allergic reactions to food ingredients, and other injuries that may occur during normal program participation. I voluntarily accept these risks on behalf of my child.

TO THE FULLEST EXTENT PERMITTED BY FLORIDA LAW, I HEREBY RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Rachel Tobol, Eran Tobol, Colossal Academy, Montessori Tides, and each of their respective directors, staff, agents, employees, and volunteers ("RELEASEES") from any and all claims, demands, losses, and liabilities arising from my child's participation in the Program, including those caused by the ordinary negligence of the RELEASEES. THIS WAIVER DOES NOT APPLY TO INJURIES CAUSED BY GROSS NEGLIGENCE OR INTENTIONAL MISCONDUCT.

Indemnification

I agree to indemnify and hold harmless Rachel Tobol, Eran Tobol, Colossal Academy, Montessori Tides, and each of their respective directors, staff, agents, employees, and volunteers from any and all claims, damages, losses, costs, and expenses arising out of or related to my child's participation in the Program.

Medical Treatment Authorization

In the event of an injury or medical emergency, I authorize the Operators to contact emergency services, provide basic first aid, and consent to emergency medical treatment on behalf of my child if I cannot be reached. I accept full responsibility for any medical expenses incurred.

Florida Law Note

Under Florida law, a parent may sign a liability waiver on behalf of a minor child. However, this waiver does not protect against claims of gross negligence or intentional misconduct. If you have questions about your legal rights, consult an attorney before signing.

Photo & Video Release

Photography & Media Consent

Cultural Explorers documents sessions through photography and video. Please select one of the following options.

Cooking Activity Acknowledgment

Food Preparation Consent

Cooking is a core part of every Cultural Explorers session. This form explains what is involved and requires your acknowledgment.

What cooking activities involve

Preparation and handling of fresh ingredients including vegetables, fruits, grains, dairy, and proteins
Use of sharp kitchen tools including knives and graters, under direct adult supervision
Use of heat sources including stovetops, ovens, and electric kettles, under direct adult supervision
Tasting of prepared foods and cultural ingredients from around the world
Handling of potential allergens including nuts, dairy, gluten, eggs, and seafood

⚠️ Parents: It is your responsibility to disclose all food allergies and dietary restrictions on Form 3. If your child has a severe allergy, contact Rachel or Eran Tobol before the first session. We are not an allergen-free facility and cross-contamination cannot be guaranteed.
